        <title id="abstract-title-a96638b9a3aa">Abstract</title>
        <p id="p-1d8e48c2e165"><bold id="strong-7">Introduction: </bold>After nearly two years, there is still no proven treatment for infection with severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2)—the virus that causes Covid-19. Currently, the two most widely known drugs for treating Covid-19 are remdesivir and favipiravir. Therefore, this study aimed to evaluate the effects of remdesivir and favipiravir on Covid-19 clinical outcomes. <bold id="strong-8">Methods: </bold>A systematic review of the literature on the PubMed and Scopus databases was undertaken to identify studies that have examined the effects of remdesivir and favipiravir on Covid-19 outcomes. To weighted group mean differences for within- and between-group comparisons, odds ratio effect sizes, and random-effects models were used. Subgroup analyses were also conducted to determine the effects of potential sources of heterogeneity, which was assessed using the I-squared (I<sup id="superscript-1">2</sup>) test. <bold id="strong-9">Results: </bold>Twenty-eight studies with a total of 10,871 adult participants were included in the analysis. According to pooled analysis results, there was no statistically significant difference between the remdesivir/favipiravir and control groups in terms of mortality, intensive care unit admissions, or adverse effects (p &gt; 0.05). Mean hospitalization duration was significantly different for those receiving remdesivir (0.1-day increase) and favipiravir (0.06-day decrease), but these findings included significant levels of publication bias. Treatment duration was found to be a significant source of heterogeneity in the mortality results. <bold id="strong-10">Conclusion: </bold>Remdesivir and favipiravir have no effect on mortality, intensive care unit admissions, or duration of hospitalization for Covid-19 patients.<bold id="strong-11"/></p>

        <bold id="strong-13">Introduction</bold>
      </title>
      <p id="p-6cb4220c3d5c">Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) was first reported in late December 2019 in Wuhan, China, and has since spread globally<bold id="s-77fb91fc8ad9"><xref id="x-afe3512012fd" rid="R194271028090578" ref-type="bibr">1</xref></bold>. By October 2021, more than 237 million people had been infected by the virus that causes Covid-19 and approximately 4.5 million people had died from their infections<bold id="s-6d6d909666b0"><xref id="x-dbdaf46921d2" rid="R194271028090579" ref-type="bibr">2</xref></bold>. The Covid-19 pandemic is an ongoing global health crisis that requires immediate attention to quickly find an appropriate treatment to reduce global mortality and morbidity associated with the disease. Currently, drugs including arbidol<bold id="s-262e75780861"><xref id="x-37645a0d311a" rid="R194271028090580" ref-type="bibr">3</xref></bold>, ribavirin<bold id="s-b370b5861e38"><xref id="x-f112473b4f95" rid="R194271028090581" ref-type="bibr">4</xref></bold>, chloroquine or hydroxychloroquine, lopinavir/ritonavir, remdesivir, and favipiravir are among those used to treat the infection experimentally. There is no known cure for SARS-CoV-2 infection<bold id="s-d422f2795785"><xref id="x-8fbbd312f682" rid="R194271028090582" ref-type="bibr">5</xref></bold>, although there are some effective treatments. Specifically, convalescent plasma<bold id="s-c5ec63d07bad"><xref id="x-455153ac8846" rid="R194271028090583" ref-type="bibr">6</xref></bold>, interleukin (IL)-1 or IL-6 inhibitors<bold id="s-b8706322c5fb"><xref id="x-cb55cff8db24" rid="R194271028090584" ref-type="bibr">7</xref></bold>, and interferons<bold id="s-1d42e77446ad"><xref id="x-f40186edad7f" rid="R194271028090580" ref-type="bibr">3</xref></bold> have been used as supportive therapy. Medications given to COVID-19 patients include antimalarial drugs such as chloroquine and hydroxychloroquine, which are also used to treat autoimmune diseases<bold id="s-dbab0f093717"><xref id="x-fbc62c83efa2" rid="R194271028090585" ref-type="bibr">8</xref></bold>, while lopinavir/ritonavir is an FDA-approved HIV treatment drug<bold id="s-b882221085c5"><xref id="x-708c28fa6bed" rid="R194271028090586" ref-type="bibr">9</xref></bold>. Gilead Science collaborated with the US Centers for Disease Control and Prevention (CDC) and the US Army Medical Research Institute of Infectious Diseases to develop remdesivir, an intravenous adenosine nucleotide analog prodrug with activity against several RNA viruses<bold id="s-3eaaa74e6c42"><xref rid="R194271028090587" ref-type="bibr">10</xref>, <xref rid="R194271028090588" ref-type="bibr">11</xref></bold>. Similarly, favipiravir is an antiviral that works against viruses containing RNA. Toyama Chemical Company was the first to approve this drug, which was used to treat influenza in Japan and China<bold id="s-e1766d569828"><xref rid="R194271028090580" ref-type="bibr">3</xref>, <xref rid="R194271028090589" ref-type="bibr">12</xref>, <xref rid="R194271028090590" ref-type="bibr">13</xref>, <xref rid="R194271028090591" ref-type="bibr">14</xref>, <xref rid="R194271028090592" ref-type="bibr">15</xref></bold>.</p>
      <p id="p-fe2de1f039bc">The Solidarity World Health Organization International Trial was a collaborative effort to find potential treatments for Covid-19 that involved 52 countries. Drugs that were investigated included remdesivir, hydroxychloroquine, lopinavir, and interferon, of which remdesivir, hydroxychloroquine, lopinavir, and interferon were found to be ineffective or have little effect for the treatment of Covid-19 hospitalized patients<bold id="s-e03ab365e649"><xref id="x-e39af11491de" rid="R194271028090593" ref-type="bibr">16</xref></bold>. In contrast, according to the findings of another review study, there was a higher rate of improvement in patients who received remdesivir than in those who received a placebo; however, there was no difference in the 14-day mortality rate<bold id="s-21a791724230"><xref id="x-d5bfd1635a33" rid="R194271028090594" ref-type="bibr">17</xref></bold>. Another review found that remdesivir significantly reduced recovery time and the occurrence of side effects, but was ineffective in treating the disease if used alone. Hence, there was improved performance when remdesivir was combined with other antiviral drugs<bold id="s-ba387287c328"><xref id="x-f5db10e907da" rid="R194271028090595" ref-type="bibr">18</xref></bold>. Favipiravir was found to be effective in treating patients with mild to moderate disease only<bold id="s-298bd0735462"><xref id="x-7fa53a9ce97a" rid="R194271028090596" ref-type="bibr">19</xref></bold>.</p>
      <p id="paragraph-12">Covid-19 is treated with antiviral drugs and supportive therapies, and numerous studies and clinical trials have been carried out to confirm the effectiveness of the drugs in combating infection. Therefore, this study aims to support the development and implementation of effective treatments for Covid-19 and analyze the results of published studies investigating the use of either remdesivir or favipiravir in COVID-19 patients to clarify their efficacy in relation to different patient outcomes.</p>
    </sec>
    <sec>
      <title id="t-af3df1bb7a35">
        <bold id="strong-14">Methods</bold>
      </title>
      <sec>
        <title id="t-d98c9f5cc128">
          <bold id="strong-15">Research Design</bold>
        </title>
        <p id="paragraph-15">This meta-analysis followed the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines<bold id="s-642bf59f13eb"><xref rid="R194271028090597" ref-type="bibr">20</xref>, <xref rid="R194271028090598" ref-type="bibr">21</xref></bold>. A quality check was conducted using the Critical Appraisal Skills Program checklist for randomized control trials (RCTs) and cohort studies<bold id="s-ece9ecaaadc7"><xref rid="R194271028090599" ref-type="bibr">22</xref>, <xref rid="R194271028090600" ref-type="bibr">23</xref></bold>.</p>
      </sec>
      <sec>
        <title id="t-65b246aeab95">
          <bold id="strong-16">Search Strategy</bold>
        </title>
        <p id="paragraph-17">Three authors independently searched the MEDLINE (PubMed) and Scopus databases for published articles. The search strategy was guided by the keywords "COVID-19," "remdesivir," and "favipiravir". A complete list of the keywords used for the search is presented in the appendix. Case-control, cohort, and RTCs were included in the searches. All of the articles were examined and there was no limitation according to study time or location. The population, intervention type, and study comparison criteria were adjusted to determine study inclusion and exclusion criteria. </p>
      </sec>
      <sec>
        <title id="t-24df9490ab7d">
          <bold id="strong-17">Inclusion and Exclusion Criteria</bold>
        </title>
        <p id="paragraph-19">The inclusion criteria were:</p>
        <p id="paragraph-20">1) Studies using case-control, cohort, or RCT designs;</p>
        <p id="paragraph-21">2) COVID-19 patients with positive laboratory tests; </p>
        <p id="paragraph-22">3) Remdesivir and/or favipiravir having been administered to the treatment/intervention group;</p>
        <p id="paragraph-23">4) Any medicines other than Remdesivir and Favipiravir in the control group; and</p>
        <p id="paragraph-24">5) Disease and treatment-related outcomes were measured.</p>
        <p id="paragraph-25">Case reports, reviews, animal research, <italic id="e-376cfe162ff5">in silico</italic> and <italic id="e-bfb76a6f03be">in vitro</italic> studies, as well as articles with full texts that were unavailable (after contacting the authors), were excluded from the study.</p>
      </sec>
      <sec>
        <title id="t-5ae8ea7f5a39">
          <bold id="strong-18">Data Extraction and Quality Control</bold>
        </title>
        <p id="paragraph-27">Two authors independently extracted data from the selected articles using a checklist. First, the titles and abstracts of identified articles were examined, and articles that were unrelated to the meta-analysis were excluded. The full texts of the remaining articles were then reviewed and included in the analysis based on the inclusion criteria. Data on the first author, year of publication, location, type of study, blinding, randomization, disease severity, sample size, type and dose of the treatment drug, population type, other treatments used, duration of treatment, age, gender, length of the follow-up period, length of the hospitalization period, recovery ratio, recovery time, mortality rate, days to first improvement, mechanical ventilation, intensive care unit (ICU) admission, ICU length of stay, acute respiratory distress syndrome, intubation, and any adverse effects in treatment and control groups were collected.</p>
      </sec>
      <sec>
        <title id="t-e827a2a34749">
          <bold id="strong-19">Statistical Analysis </bold>
        </title>
        <p id="paragraph-29">We used a proposed estimation model<bold id="s-24441536c896"><xref id="x-497e42e722aa" rid="R194271028090601" ref-type="bibr">24</xref></bold> to justify the scale and outcome indicators (median, interquartile range (IQR), mean and standard deviation (SD)). We anticipated significant heterogeneity among the studies and, therefore, used a random effects model. To examine the heterogeneity of the effect-size estimates among the studies, the Q-statistic, its p-value, a forest plot, and I<sup id="s-8d2fbbf747cd">2</sup> were used. The Q-statistic was used to compare the observed and expected effect size dispersions across the studies, and the p-values for statistical significance are provided. The I<sup id="s-3329fa515311">2</sup> value is the ratio of real to observed heterogeneity. I<sup id="s-0e9fdb3202fd">2</sup> values between 0% and 50% were considered to be acceptable heterogeneity, while values greater than 50% were considered to indicate significant heterogeneity<bold id="s-186753995838"><xref id="x-88f4b262cf86" rid="R194271028090602" ref-type="bibr">25</xref></bold>. Subgroup analysis and meta-regression were used to determine the sources of heterogeneity when it was significant<bold id="s-50840c4de36c"><xref id="x-367b68737a3f" rid="R194271028090603" ref-type="bibr">26</xref></bold>. A funnel plot and Egger's regression test were used to evaluate publication bias (given the low power of the test, a = 0.1 was used)<bold id="s-882a7c310d1e"><xref id="x-26a6c88179d1" rid="R194271028090604" ref-type="bibr">27</xref></bold>. Stata Statistical Software Version 15.1. (StataCorp LP., College Station, TX, USA) was used for all analyses.</p>

          <bold id="strong-21">Systematic Search and Characteristics of the Included Studies </bold>
        </title>
        <p id="paragraph-32">The initial search uncovered 8,329 relevant records of which 3,561 duplicates were removed. After screening the titles and abstracts, 633 studies were considered eligible for further screening. Next, the full texts of the studies were assessed and 28 studies with a total of 10,871 adult participants were found to be eligible for inclusion in the meta-analysis (<bold id="s-475ec40b2394"><xref id="x-3dfb604dfca4" rid="f-8f5969373d9b" ref-type="fig">Figure 1</xref></bold>).</p>
      </sec>
      <sec>
        <title id="t-5db8faba6f36"><bold id="strong-22">Pooled Analysis of Covid-19 Outcomes After Receiving Remdesivir or Favipiravir</bold> </title>
        <p id="paragraph-34">As the included studies reported their outcomes differently, event counts rather than percentages and proportions were used. For mortality rates, some studies used counts while others used ORs. Therefore, an analysis was conducted for both indicators after converting the counts into ORs and 95% CIs. Hospitalization duration was also measured using the mean indicator and is presented as mean differences.</p>
        <sec>
          <title id="t-c944a5da9870">
            <bold id="strong-23">Mortality Rate</bold>
          </title>
          <p id="paragraph-36">According to the results of a pooled analysis of 17 studies, there was no statistically significant difference in the mortality rate between the remdesivir and control groups (p: 0.493). Similarly, based on the results of a pooled analysis of 8 studies, there was no significant difference in the mortality rate between the favipiravir and control groups (p: 0.774). Heterogeneity was high (&gt; 50%) for all of the studies; although, no publication bias was observed (Egger’s test p-value &gt; 0.20) (<bold id="s-42eb462e3788"><xref id="x-46c27b01a1d4" rid="tw-5a45633afdf8" ref-type="table">Table 2</xref></bold>, <bold id="s-c38fb53cffb0"><xref rid="f-2f7f4ed608f0" ref-type="fig">Figure 2</xref>, <xref rid="f-953f59ccc79b" ref-type="fig">Figure 3</xref></bold>).<bold id="strong-24"/></p>

        <sec>
          <title id="t-c1a45a03dd3f">
            <bold id="strong-25">Admission to the ICU</bold>
          </title>
          <p id="paragraph-38">Results from a pooled analysis of 3 studies using remdesivir and 4 studies using favipiravir<bold id="s-5691b8ce1353"><xref rid="R194271028090607" ref-type="bibr">30</xref>, <xref rid="R194271028090608" ref-type="bibr">31</xref>, <xref rid="R194271028090614" ref-type="bibr">37</xref>, <xref rid="R194271028090621" ref-type="bibr">44</xref>, <xref rid="R194271028090622" ref-type="bibr">45</xref>, <xref rid="R194271028090624" ref-type="bibr">47</xref>, <xref rid="R194271028090627" ref-type="bibr">50</xref>, <xref rid="R194271028090631" ref-type="bibr">54</xref></bold> found no statistically significant differences in ICU admission outcomes between the intervention and control groups (p-value for remdesivir: 0.785, p-value for favipiravir: 0.483). The heterogeneity was high (&gt; 50%) and significant, but no publication bias was found (<bold id="s-a565e6cc1fb2"><xref id="x-39e30431adfc" rid="tw-5a45633afdf8" ref-type="table">Table 2</xref></bold>, <bold id="s-f64a9968a602"><xref rid="f-0fe6ce912b2b" ref-type="fig">Figure 4</xref>, <xref rid="f-6b4a24d92d4d" ref-type="fig">Figure 5</xref></bold>).<bold id="strong-26"/></p>

            <bold id="strong-27">Adverse Effects</bold>
          </title>
          <p id="paragraph-40">A pooled analysis of 4 studies<bold id="s-daf86abe8453"><xref rid="R194271028090610" ref-type="bibr">33</xref>, <xref rid="R194271028090617" ref-type="bibr">40</xref>, <xref rid="R194271028090625" ref-type="bibr">48</xref></bold> indicated that patients receiving remdesivir had no significantly higher adverse effects compared to control groups (p: 0.732). While the heterogeneity was both high (&gt; 50%) and significant, no publication bias was found. This analysis was not possible for favipiravir due to the low number of available, published studies (&lt; 3) (<bold id="s-9061e20859e3"><xref id="x-1e339c39af03" rid="tw-5a45633afdf8" ref-type="table">Table 2</xref></bold>,<bold id="s-198af29d35e1"><xref rid="f-6b4a24d92d4d" ref-type="fig">Figure 5</xref>, <xref rid="f-fc78efd82ee6" ref-type="fig">Figure 6</xref></bold>).<bold id="strong-28"/></p>

          <p id="paragraph-42">The pooled analysis for hospitalization duration consisted of 3 studies<bold id="s-5a9281ee02c0"><xref rid="R194271028090611" ref-type="bibr">34</xref>, <xref rid="R194271028090612" ref-type="bibr">35</xref>, <xref rid="R194271028090625" ref-type="bibr">48</xref></bold>, which showed that the use of remdesivir significantly increased hospitalization duration in the intervention groups by 0.1 days (p: 0.000). In contrast, the results of an analysis of 3 studies<bold id="s-72a4ebac58df"><xref rid="R194271028090608" ref-type="bibr">31</xref>, <xref rid="R194271028090614" ref-type="bibr">37</xref>, <xref rid="R194271028090615" ref-type="bibr">38</xref></bold> that used favipiravir showed significantly reduced hospitalization duration (by 0.06 days compared to the control groups (p: 0.019)). However, high heterogeneity (&gt; 50%) and publication bias were observed (<bold id="s-9bdc337ba0b1"><xref id="x-5d3f0b0db6db" rid="tw-5a45633afdf8" ref-type="table">Table 2</xref></bold>, <bold id="s-74bd982a49e4"><xref rid="f-c3566990c5c4" ref-type="fig">Figure 8</xref>, <xref rid="f-8482e404ccd1" ref-type="fig">Figure 9</xref></bold>).</p>
          <p id="paragraph-43">The mortality rate in different subgroups was not significantly different between the intervention and control groups (<bold id="s-5169705205dd"><xref id="x-43bc2e6b09c5" rid="tw-cf927f2e6196" ref-type="table">Table 3</xref></bold>). The subgroups analyzed in this meta-analysis included study design, treatment duration (median: 7 days), and age (median: 59 years). Analyzing other outcomes was not possible due to the lack of studies reporting on each possible subgroup variable.</p>
          <p id="paragraph-44"><bold id="s-5c59d506e401"><xref id="x-2450821a4635" rid="tw-8a2c3a96c055" ref-type="table">Table 4</xref></bold> presents the possible sources of the high heterogeneity observed in the analysis. The only variable that significantly effected heterogeneity was treatment duration, which was significant for both remdesivir and favipiravir. Further analysis was not possible for the other outcomes due to the lack of published studies reporting on the different subgroup variables. </p>
        </sec>
      </sec>

    <sec>
      <title id="t-2f5e051fc2b7">
        <bold id="strong-30">Discussion</bold>
      </title>
      <p id="paragraph-46">This study aimed to evaluate the effectiveness of two well-known drugs, remdesivir and favipiravir, for treating Covid-19 infection. Remdesivir was introduced as an effective drug for the treatment of Covid-19 after obtaining its first emergency use authorization in May 2020 in the United States and then later in Japan. However, its use has had many critics<bold id="s-de8416ee78a0"><xref id="x-a02201fe1a1f" rid="R194271028090633" ref-type="bibr">56</xref></bold>. Unfortunately, despite both the passage of time and an increase in the number of observational studies and RCTs, questions regarding the efficacy of these drugs remain unanswered primarily because the results have been controversial and heterogeneous between the various investigations. One way to address this issue is to conduct systematic reviews and meta-analysis studies.</p>
      <p id="paragraph-47">As a ribonucleotide analog and selective inhibitor of the viral RNA polymerase enzyme, favipiravir performs a wide range of antiviral activities against RNA-carrying viruses, which includes blocking viral genome replication and transcription. In Japan and China, favipiravir is licensed for the treatment of novel influenza viruses. It is also effective against Ebola and other RNA-based viruses that cause hemorrhagic fevers<bold id="s-c19ab8555eb2"><xref id="x-d5822ab57412" rid="R194271028090612" ref-type="bibr">35</xref></bold>. However, according to the findings of the current meta-analysis, favipiravir had no significant effect on reducing the mortality rate or ICU admissions in Covid-19 patients. Other meta-analyses have also found that the drug does not decrease many of the indicators associated with Covid-19, including death, hospitalization duration, transfer to the ICU, <italic id="e-18da7f85faca">etc</italic>.<bold id="s-da397482e498"><xref rid="R194271028090634" ref-type="bibr">57</xref>, <xref rid="R194271028090635" ref-type="bibr">58</xref>.</bold> However, in this study, favipiravir was found to reduce hospitalization duration by 0.06 days. Although this reduction was statistically significant, it is clinically equivalent to approximately 86 minutes, which would not be considered important from a patient perspective. We also found that favipiravir administration did not induce more side effects compared with controls. This result is consistent with those from another meta-analysis<bold id="s-5c0d2aaa48e0"><xref id="x-283802833cbc" rid="R194271028090636" ref-type="bibr">59</xref></bold>.  However, few interventional and secondary studies using favipiravir have been conducted, and finding high-quality interventional studies with large sample sizes is challenging. Furthermore, the high heterogeneity in the results suggests substantial variation in the target parameters of these studies.</p>
      <p id="paragraph-48">Remdesivir is an adenosine nucleotide analogue prodrug that inhibits viral replication by inducing chain termination in the RNA-dependent RNA polymerase enzyme of SARS-CoV-2<bold id="s-5073da3d8120"><xref id="x-b00ae9168893" rid="R194271028090637" ref-type="bibr">60</xref></bold>. However, there are debates concerning the effectiveness of remdesivir, and studies, including meta-analyses, have not yet reached a consensus regarding its efficacy. According to some of the articles used in the current study, the use of remdesivir did not effect the mortality or ICU admission rates in Covid-19 patients. In contrast, others have found that the use of the drug reduced the mortality rate by 34% (OR: 0.66). These inconsistent results have been found in other meta-analyses as well<bold id="s-483d57d1f558"><xref rid="R194271028090639" ref-type="bibr">61</xref>, <xref rid="R194271028090638" ref-type="bibr">62</xref>, <xref rid="R194271028090640" ref-type="bibr">63</xref></bold>. We also found that taking remdesivir increased treatment duration by 0.1 days (144 minutes). While this finding is not consistent with other, similar studies that have found remdesivir neither changes hospitalization duration<bold id="s-033d6b81ce87"><xref id="x-85ac4ce98c71" rid="R194271028090639" ref-type="bibr">61</xref></bold> nor reduces it<bold id="s-a960f1ba9698"><xref rid="R194271028090638" ref-type="bibr">62</xref>, <xref rid="R194271028090640" ref-type="bibr">63</xref></bold>, it should be noted that there was both high heterogeneity and publication bias, both of which could have affected our findings. The effect of the treatment duration variable in heterogeneity should be also taken into account. Specifically, if only this variable is considered, it is possible to assume that treatment duration changes remdesivir’s efficacy. </p>
      <p id="p-1f231826020f">One limitation of this study (and similar meta-analyses) is the severe lack of high-quality, interventional studies with appropriate sample sizes, sufficient follow-up periods, and similar treatment protocols needed to reduce heterogeneity. One of the strengths of this study was the simultaneous review of observational and interventional studies as well as sub-group analyses of different outcome variables.</p>
    </sec>
    <sec>
      <title id="t-238d3fd39d68">
        <bold id="strong-31">Conclusion</bold>
      </title>
      <p id="paragraph-51">Based on the results of this meta-analysis, both remdesivir and favipiravir have very slight or no effect on mortality rates, ICU admissions, or hospitalization duration in Covid-19 patients. However, more vigorous interventional studies are needed before coming to firm conclusions about the effects of these drugs on covid-19 patient outcomes. </p>
