        <title id="abstract-title-71e97b82ac8c">Abstract</title>
        <p id="p-9e0187763b6c"><bold id="strong-2">Background:</bold> Colorectal cancer is one of the most common malignancies with a high rate of liver metastasis, and various gene mutations have been proven to be involved in its pathogenesis. One of them is <italic id="e-009b3cca3c87">BRAF</italic> gene mutations, the most frequent of which—V600E—has a poor prognosis in patients with colon cancer. <bold id="strong-3">Case presentation:</bold> By applying the FOLFIRINOX regimen and a specific maintenance strategy containing vemurafenib, we achieved disappearance of metastatic lesions, no recurrence, and good stability in a Kurdish patient with metastatic colon cancer harboring <italic id="emphasis-2">RAS-wt/BRAF</italic> V600E. <bold id="strong-4">Conclusion:</bold> Our therapeutic strategy allows the selection of the most appropriate treatment approach for patients.</p>

      <title id="t-64cfd5a88b6e">Introduction</title>
      <p id="p-03a9f1da522f">Colorectal cancer (CRC) is the second leading cause of cancer death and the third most common cancer worldwide<bold id="s-2a19dceb0967"><xref id="x-c2634db6fff4" rid="R154096526048976" ref-type="bibr">1</xref></bold>. Distant metastases occur in approximately 65% of cases. The liver is the most common site of CRC metastasis (20 — 30%), followed by the lungs (10 — 25%)<bold id="s-bded0ad791bd"><xref rid="R154096526048977" ref-type="bibr">2</xref>, <xref rid="R154096526048978" ref-type="bibr">3</xref></bold>. The survival rate is lower in men than in women, while the 5-year survival rate is poorer for rectal cancer than for colon cancer<bold id="s-41a1bad289fb"><xref id="x-eeb047830b09" rid="R154096526048979" ref-type="bibr">4</xref></bold>. Generally, CRC affects 71% of the colon and 29% of the rectum<bold id="s-53673a780f51"><xref id="x-f910a47debdf" rid="R154096526048980" ref-type="bibr">5</xref></bold>. Sporadic cases are observed in approximately 70% of patients, and environmental and dietary factors are mainly involved. Hereditary predisposition is noted in &lt; 10% of patients and occurs owing to several genetic changes, which can be identified through specific genetic tests<bold id="s-defa7424d38c"><xref rid="R154096526048980" ref-type="bibr">5</xref>, <xref rid="R154096526048981" ref-type="bibr">6</xref></bold>. </p>
      <p id="p-143bd4b1204d">Based on the different molecular mechanisms involved in CRC, many treatment methods can be used. The most important molecular mechanisms underlying different subtypes of CRC are microsatellite instability, chromosomal instability, and DNA repair machinery error<bold id="s-5c1e495feb7c"><xref id="x-36c3e5036bc6" rid="R154096526048982" ref-type="bibr">7</xref></bold>. As a serine/threonine kinase, v-raf murine sarcoma viral oncogene homolog B1 (<italic id="emphasis-4">BRAF</italic>) is located downstream of the mitogen-activated protein kinase (MAPK)/epidermal growth factor receptor (EGFR) signaling pathway<bold id="s-3afaa21292a4"><xref id="x-9453ab82193b" rid="R154096526048983" ref-type="bibr">8</xref></bold>. The V600E mutation of this gene permanently activates the MAPK pathway and is associated with epigenetic activation of MLH1, resulting in the microsatellite instability phenotype in patients with CRC<bold id="s-db9b601c4723"><xref rid="R154096526048984" ref-type="bibr">9</xref>, <xref rid="R154096526048985" ref-type="bibr">10</xref></bold>. This mutation occurs in a specific subtype of CRC with a poor prognosis. After performing molecular and diagnostic tests, we determined that our patient had <italic id="emphasis-5">RAS-wt</italic>/<italic id="emphasis-6">BRAF</italic> V600E mutation; we immediately treated her with the FOLFIRINOX regimen and a specific maintenance therapy and obtained promising results.</p>
    </sec>
    <sec>
      <title id="t-182af6fc2f4a">CASE PRESENTATION</title>
      <p id="p-bb16c0e9da87">In February 2019, a 45-year-old Kurdish woman was referred to the Kermanshah Hematology and Oncology Clinic with symptoms of weight loss and abdominal pain. The patient had no relevant clinical history. Following a physical examination of the patient, the physician requested an abdominal CT. Laboratory examination indicated an increase in the CEA and CA19-9 levels and a decrease in the RBC indices. The complete blood count (CBC) testing results were as follows: platelet count: 284 (normal range: 120 <bold id="strong-9">– </bold>450) × 10<sup id="superscript-9">9</sup>/L, hemoglobin level: 9.1 (normal range: 11.5 <bold id="strong-10">– </bold>16.5) g/dL, and white blood cell (WBC) count: 6.6 (normal range: 3.5 <bold id="strong-12">– </bold>11) × 10<sup id="superscript-10">9</sup>/L. CT illustrated a fungating tumor with a diameter of 40 × 26 mm seen in the transverse colon. Subsequently, PET-CT displayed primary lesions as mural thickening in the splenic flexure of the transverse colon with mesenteric lymph node<bold id="strong-15"> </bold>nvolvement and multiple metastatic lesions on the right side of the liver. A panel of genetic tests, including tests for <italic id="emphasis-7">KRAS</italic>,<italic id="emphasis-8"> NRAS</italic>,<italic id="emphasis-9"> PLD1</italic>, and <italic id="emphasis-10">BRAF</italic> mutations, was performed; only <italic id="emphasis-11">BRAF</italic> mutation (V600E) tested positive. Based on these findings, the patient was diagnosed with disseminated colon cancer and treated with 12 cycles of the FOLFIRINOX regimen (leucovorin, 5-fluorouracil [FU], irinotecan, and oxaliplatin) and cetuximab (Erbitux) therapy. The metastatic lesions resolved within 2 weeks after the chemotherapy courses. Thereafter, total colectomy and hyperthermic intraperitoneal chemotherapy were conducted in the transverse colon, and the tumor lesions were removed.</p>
      <p id="p-3402ab26f05b">Pathological examination did not show any tumors. Owing to the good<bold id="strong-19"> </bold> response and poor prognosis of <italic id="emphasis-12">BRAF</italic> mutation in the colon tumor, the patient received a maintenance therapy regimen consisting of capecitabine, vemurafenib, and Erbitux for more than 2 years.<bold id="strong-21"> </bold> During this time, the initial response to maintenance therapy was relatively good, and the CBC testing results were as follows: platelet count: 150 × 10<sup id="superscript-11">9</sup>/L, hemoglobin level: 11.6 g/dL, and WBC count: 5.4 × 10<sup id="superscript-12">9</sup>/L. Similarly, there was no evidence of residual invasive tumor in the colon pathology report. The patient currently receives continual treatment with gradual tapering of these combinations and is followed up regularly. </p>

        <bold id="s-1cb39c8f6782">DISCUSSION</bold>
      </title>
      <p id="paragraph-12">The <italic id="emphasis-14">BRAF</italic> gene has 18 exons, and its gene locus is on chromosome 7 and functions as a proto-oncogene<bold id="s-bbb1094c5521"><xref id="x-e80baa3f327d" rid="R154096526048986" ref-type="bibr">11</xref></bold>. This serine/threonine protein kinase is an important signaling molecule in the EGFR-mediated MAPK pathway, the activation of which is mediated by GTPase/RAS<bold id="s-d31ec108a113"><xref id="x-0ce80357f964" rid="R154096526048983" ref-type="bibr">8</xref></bold>. This pathway and the closely related phosphatidylinositol 3-kinase/AKT signaling pathway play a substantial role in the transcription of genes involved in cancer progression. Therefore, abnormalities involved in the activation of these pathways are among the causes of many types of human cancer<bold id="s-2886c2977996"><xref id="x-236ed6ef2c96" rid="R154096526048987" ref-type="bibr">12</xref></bold>. Approximately 70% of <italic id="emphasis-15">BRAF</italic>-activating mutations occur in malignant melanomas, and the most common mutation is V600E, which changes valine to glutamic acid in codon 600 and accounts for approximately 95% of mutations in this gene<bold id="s-89ce6ab1e12c"><xref id="x-910e6a4b8c93" rid="R154096526048988" ref-type="bibr">13</xref></bold>. This substitution has been observed with a lower frequency in other types of human cancer, including CRC at 10 – 20%<bold id="s-41fc59c40840"><xref id="x-d2060fed3199" rid="R154096526048989" ref-type="bibr">14</xref></bold>. As revealed in a recent meta-analysis, tumors with <italic id="emphasis-16">BRAF</italic> V600E mutations occur mainly on the right side of the colon, with a higher prevalence in women and elderly populations (over 60 years of age)<bold id="s-814273a63dc0"><xref id="x-586b6eb8e48f" rid="R154096526048990" ref-type="bibr">15</xref></bold>.</p>
      <p id="paragraph-13">Notably, there is a mutually exclusive relationship between <italic id="emphasis-17">KRAS</italic> mutation and <italic id="emphasis-18">BRAF</italic> V600E<bold id="s-cfa5ccc57aad"><xref id="x-fa78a86f4081" rid="R154096526048991" ref-type="bibr">16</xref></bold>. Accordingly, the treatment strategies for patients can differ. In patients with <italic id="emphasis-19">KRAS-wt</italic>, the recommended first-line treatment is the administration of FOLFOX or FOLFIRI regimens, cetuximab or panitumumab, and doublet chemotherapy. These may be considered for some patients with right-sided <italic id="emphasis-20">RAS-wt</italic> tumors to reduce cytotherapy<bold id="s-ef14e47698fa"><xref id="x-9a63c8afdc7e" rid="R154096526048992" ref-type="bibr">17</xref></bold>. </p>
      <p id="paragraph-14">In our study, the patient had <italic id="emphasis-21">RAS-wt/BRAF</italic> V600E mutation, which was observed with transverse colon involvement and liver metastasis. Thus, we started treatment with FOLFIRINOX (FFX) plus cetuximab. FOLFIRINOX has been recommended and extensively prescribed as a first-line treatment for patients with pancreatic ductal adenocarcinoma since 2011<bold id="s-78d890154e89"><xref id="x-98c467470708" rid="R154096526048993" ref-type="bibr">18</xref></bold>. FFX includes leucovorin, FU, irinotecan, and oxaliplatin<bold id="s-b96e5287ccd3"><xref id="x-e297a3cedbce" rid="R154096526048994" ref-type="bibr">19</xref></bold>, and this regimen plus cetuximab can be used in the first-line treatment of patients with metastatic CRC<bold id="s-1a53d6001f98"><xref id="x-a1b20ca00ff5" rid="R154096526048995" ref-type="bibr">20</xref></bold>. After 12 cycles of this regimen, the liver metastatic lesions of our patient resolved, and colectomy was then performed. Because of the favorable response and poor prognosis of <italic id="emphasis-22">BRAF</italic> V600E mutations in the colon tumor, the patient received maintenance therapy, which included capecitabine (chemotherapy drug), vemurafenib (BRAF inhibitor), and cetuximab (EGFR inhibitor) and continued for more than 2 years; to date, the results have been promising. </p>
      <p id="paragraph-15">Maintenance therapy generally consists of a less toxic portion of the previous regimen or an entirely disparate combination to which the patient has not formerly been exposed<bold id="s-26cc18b6c4eb"><xref id="x-3eb12a819beb" rid="R154096526048995" ref-type="bibr">20</xref></bold>. This therapeutic strategy is performed in patients who have not responded to salvage therapy and is intended to extend the positive results achieved with first-line induction treatment<bold id="s-1523c4092295"><xref id="x-da9b3028fd00" rid="R154096526048996" ref-type="bibr">21</xref></bold>. Treatment continuation or medication break is left to the doctor’s discretion. The advantages of drug vacations or therapeutic breaks include less toxicity, fewer hospital visits, better quality of life, and cost saving<bold id="s-1742bcad5b5d"><xref rid="R154096526048995" ref-type="bibr">20</xref>, <xref rid="R154096526048997" ref-type="bibr">22</xref></bold>. </p>

      <title id="t-f6b823bccb73">CONCLUSION</title>
      <p id="paragraph-18">The use of FOLFIRINOX for complete response and resolution of metastatic lesions in patients with colon cancer with <italic id="emphasis-23">RAS-wt/BRAF</italic> V600E mutation is rarely reported. Our study showed that this combination plus a maintenance therapy strategy can lead to a complete and sustained therapeutic response. This strategy allows the selection of the most appropriate therapeutic method for the patient, yielding benefits such as cost savings and fewer hospital referrals. Our findings emphasize the importance of further investigations through larger prospective studies.</p>
