        <title id="abstract-title-30080b1fe4dd244e82da80279c889397">Abstract</title>
        <p id="paragraph-0af0a143c8eaff6182341607f9cd5f97"><bold id="strong-1">Background:</bold> Chromosome Xq11-12 is the place that the androgen receptor (AR) sequence appears. Herein, the prevalence of this biomarker and its relation with pT stage and tumor grade was reported. <bold id="strong-2">Methods: </bold>Four online sites (PubMed, Scopus, Web of Science, and Cochrane Library) have been searched up to Sep 2018 systematically. Meta-Analysis software version 2.0 (CMA 2.0) and STATA 14.0 statistical software were utilized. Publication bias did not exist. <bold id="strong-3">Results:</bold> From the initial 1141 articles identified from the systematically searches. At last, nine of them remained for analysis. The meta-analysis included 1447 patients that 345 of them had AR expression. AR expression significantly correlated with low tumor grade and low tumor pT stage. <bold id="strong-4">Conclusion:</bold> AR expression was 28.2%, and it had the relationship with tumor low grade and low pT stage. Additional studies required to figure out the role of it on RCC patients.</p>

      <p id="paragraph-832da6d3399b5273ae336ba56e8671d7"> Renal cell carcinoma (RCC) is the foremost different urological malignancy among adults <xref id="xref-ebfde8c97b6d911f7da77490855a98ba" rid="357780:7906671" ref-type="bibr">1</xref>. The most prevalent form of malignancy RCC is clear cell RCC (ccRCC) and represents over 90% of malignant kidney tumors <xref id="xref-df67a0f964c179bc5f32c450095729a4" rid="357780:7906655" ref-type="bibr">2</xref>. The androgen receptor (AR) sequence is found at chromosome Xq11-12 and includes of eight exons <xref id="xref-40600622510beed260115101fbb280c7" rid="357780:7906652" ref-type="bibr">3</xref>. Prevalence of the AR is appeared in several parts of the body, principally in sexual parts of men. Also, it was reported in breast, bladder, liver, gastrointestinal <xref id="xref-513243afd9ebe1e65b8e278167f0ecea" rid="357780:7906658" ref-type="bibr">4</xref>. High AR expression is potential prognosis factor in bladder cancer <xref id="xref-f1615a90923f0ce16f6185d66e0dfba1" rid="357780:7906665" ref-type="bibr">5</xref>, and it created longer life in patients with serous carcinoma of the ovary <xref id="xref-714a453b8990c7f8820fe5d7f67c9061" rid="357780:7906667" ref-type="bibr">6</xref>, an agent for progressive the squamous cell carcinoma of head and neck <xref rid="357780:7906672" ref-type="bibr">7</xref><xref rid="357780:7906678" ref-type="bibr">8</xref>. We tend to are responsive to only rare studies evaluating AR immunohistochemical staining in RCC <xref rid="357780:7906661" ref-type="bibr">9</xref><xref rid="357780:7906668" ref-type="bibr">10</xref><xref rid="357780:7906682" ref-type="bibr">11</xref>. Though the targeted therapy creates an effective route for advanced RCC patients <xref id="xref-9902ec204fc57440878e0f509b475f49" rid="357780:7906724" ref-type="bibr">12</xref>, the relationship between AR expression and RCC progression stay uncertain until now <xref id="xref-1100ff5c4c3769f0ce936a1cd9b3fda0" rid="357780:7906669" ref-type="bibr">13</xref>. This meta-analysis reports the expression of AR and its relation with some pathological factors in RCC.</p>

    <sec>
      <title id="title-24ecfd01b7458c78a33c4df795523429">Methods</title>
      <sec>
        <title id="title-5b0759093de42a22f193877a1cbec60d">Search Strategy</title>
        <p id="paragraph-c95069d2bd6a34d55e0ff05f13b7f004">PubMed, Scopus, Web of science, and Cochran library included in this study. Search terms included systematically up to September 2018 were “androgen receptor”, “AR”, “kidney”, “renal cell carcinoma”, “cancer”, “carcinoma”, and “tumor. The studies were searched for the assessment of expression of the AR in RCC patients in English abstract.</p>
      </sec>
      <sec>
        <title id="title-c1a02720b5e998aeb6d71e37b26c4152">
          <bold id="strong-06164b06ae92997017bdefa49cf2d729">Inclusion and Exclusion Criteria</bold>

          <p id="paragraph-e97d20ad18102e7f055a6048ed5f032e">1) Case-control, case report, and review studies; conference paper and letter to editor</p>
          <p id="paragraph-fc2af5e5dbf7efdc948c2ac462fffef7">2) The study with incomplete data</p>
        </sec>
      </sec>
      <sec>
        <title id="title-db24442abc44f3f1eb4e59f69b9bf878">Data extraction</title>
        <p id="paragraph-9d903d24653fc4c1523987ec3d1792da">First author, year of publication, nation, RCC samples, expression, grade, pT stage, number of males, and the mean age were extracted. We could not have analysis on types of survival as a result of the studies didn't have the same data.</p>
      </sec>
      <sec>
        <title id="title-aeed8272cbb61118792de5bd788846f0">Statistical analysis</title>
        <p id="paragraph-e22ecf7a4480b4703b52119d7862061b">Comprehensive Meta-Analysis software version 2.0 (CMA 2.0) and STATA 14.0 statistical software (StataCorp LP, College Station, TX, USA) were used for random-effect analysis. P-value (2-sided) &lt;0.05 is significant.</p>
        <sec>
          <title id="title-065e2533a2f37a4589fd1ef87f899c9d">CMA 2.0</title>
          <p id="paragraph-a956dad5b9e1390b48f2aa482776f573">The event rate (ER) with 95% confidence interval (95% CI) was calculated for estimation of the expression of AR mutations in RCC patients. Begg’s and Egger’s tests were calculated for bias.</p>
        </sec>
        <sec>
          <title id="title-ee2701d73f24c4538407ef2c5855be61">STATA 14.0</title>
          <p id="paragraph-11">P&lt;0.1 is significant for heterogeneity. Heterogeneity is assessed by the Q<x>
</x> and I 2 statistics.<x>

        </sec>
      </sec>
    </sec>
    <sec>
      <title id="title-317e8585206562613811e157264f57d4">Results</title>
      <p id="paragraph-bdab95b9c7c4003ee011ee259938e656">After primary searches, 1141 articles identified. Twenty one studies remained after we excluded the non-relevant studies. Then, twelve studies were removed based on reasons (five conference papers, three reviews, two articles without full text, one letter to editor, and one database). At last, nine of them remained for analysis (<bold id="strong-ef968a5c84194b847e2749e7f0579860"><xref id="xref-18fae1115f6e7b43ee780fc0e34ea108" rid="figure-86cba4cdffb630d00e7e9cb680355a3d" ref-type="fig">Figure 1</xref></bold>).</p>

One hundred seventy-one cases in four studies <xref rid="357780:7906661" ref-type="bibr">9</xref><xref rid="357780:7906668" ref-type="bibr">10</xref><xref rid="357780:7906682" ref-type="bibr">11</xref><xref rid="357780:7906664" ref-type="bibr">18</xref> of meta-analysis study were male, and 59 cases were female (sum=202). Mean age (min-max) in two studies was 60.06 (29-82). Three studies <xref rid="357780:7906661" ref-type="bibr">9</xref><xref rid="357780:7906682" ref-type="bibr">11</xref><xref rid="357780:7906657" ref-type="bibr">14</xref><xref rid="357780:7906664" ref-type="bibr">18</xref> on pT stage in 132 patients with expression AR showed 51 cases pT stage 1, 20 cases pT stage 2, 41 cases pT stage 3. And also, 160 low grade versus 42 high-grade cases exist in four studies <xref rid="357780:7906661" ref-type="bibr">9</xref><xref rid="357780:7906668" ref-type="bibr">10</xref><xref rid="357780:7906682" ref-type="bibr">11</xref><xref rid="357780:7906664" ref-type="bibr">18</xref> (<bold id="strong-1beeb990e8ce2202f7d660ad5482cae0"><xref id="xref-b970be836354e12e7215c7efbff79191" rid="table-wrap-37407bb8bea2b63d63c3f9fb6e8c661a" ref-type="table">Table 1</xref>)</bold>.</p>

      </title>
      <p id="paragraph-39">Radiotherapy, chemotherapy, and immunotherapy used for treat metastatic RCC patients <xref id="xref-541fe2e362d1152f80587a790f3f4b22" rid="357780:7906724" ref-type="bibr">12</xref>. But, a lot of controversial for treatment of this malignancy are existed until now <xref rid="357780:7906669" ref-type="bibr">13</xref><xref rid="357780:7906659" ref-type="bibr">17</xref>. Moreover, AR expression has been related to chemo-responsiveness <xref id="xref-834066647bd596fe9150357d44a524c3" rid="357780:7906656" ref-type="bibr">20</xref>. Exaggerated AR expression was related to attenuate responsiveness and exaggerated migration of tumor cells <xref id="xref-0aff8c3bfcf484f71b254b57d748e72f" rid="357780:7906676" ref-type="bibr">21</xref>. The clear cell RCC resists chemotherapy and radiation with a restricted therapeutic period of the anti-angiogenesis targeted therapy (6–15 months) <xref id="xref-d3d2177c9676553afe7ae3516c68650f" rid="357780:7906673" ref-type="bibr">22</xref>. Clinical researchers have disclosed that some patients reply to endocrine therapy objectively or subjectively <xref rid="357780:7906670" ref-type="bibr">23</xref><xref rid="357780:7906679" ref-type="bibr">24</xref>. AR can help to the pathobiology of breast malignancy <xref id="xref-beb653579ab125a9a522c987197d36d3" rid="357780:7906666" ref-type="bibr">25</xref>, and maybe inhibition of androgen sign had a therapeutic role in it <xref id="xref-1fb9be6aa772fa846eab2ba6e025b904" rid="357780:7906650" ref-type="bibr">26</xref>. Firstly, AR was observed in cancerous renal tissues and then appears in other urinary organs like prostate and then breast cancer <xref id="xref-dc0b0055f11f99bef221eb7f28fdae62" rid="357780:7906674" ref-type="bibr">27</xref>. Nakano<italic id="emphasis-816ef3ada99d4df9c431df3bc92258ee"> et al</italic>., who noted that patients with RCC showing immunoreactivity for one or a lot of hormone receptors (ER, PR and/or AR) had a considerably higher survival rate <xref id="xref-43f170250015329006f2b807fb9e6eeb" rid="357780:7906664" ref-type="bibr">18</xref>. A cohort found that AR expression in clear cell RCC was prognostic which high expression <xref id="xref-eec7973711d62b658bde022452fba4fb" rid="357780:7906681" ref-type="bibr">28</xref>. Cut off for express AR reported from 12.9% to 100% <xref rid="357780:7906660" ref-type="bibr">29</xref><xref rid="357780:7906675" ref-type="bibr">30</xref><xref rid="357780:7906663" ref-type="bibr">31</xref>. AR expression found in non-invasive urothelial malignancies as compared to invasive urothelial malignance <xref rid="357780:7906651" ref-type="bibr">32</xref><xref rid="357780:7906677" ref-type="bibr">33</xref>. </p>
      <p id="paragraph-40">Consequently, AR immunoreactivity was related to renowned favorable prognostic factors, like tiny tumor size, low pT stage (more than 30% in pT1a tumors) in addition as low histological grade <xref id="xref-33a7710cc1f0edb46c8ba28de3f4f1fe" rid="357780:7906662" ref-type="bibr">34</xref>. The AR-positive rate ranged from 16.3%-44.3% in RCC tissues in this study. Noh <italic id="emphasis-aa48140961353fd665163f16e76cdd49">et al</italic>., <xref id="xref-d70c01604a114444af40b5934b60f698" rid="357780:7906668" ref-type="bibr">10</xref> and Concolino <italic id="emphasis-053d0fef5b2c57412a4478629173f49d">et al</italic>., <xref id="xref-613a53109e68dbde6bb8f62947b7e8b5" rid="357780:7906654" ref-type="bibr">16</xref> showed the level of AR-positive is higher than our meta-analysis. In other hands Langne<italic id="emphasis-1b67b10c8158670bc94a68da2c73b826"> et al</italic>., <xref id="xref-29a29e3bacd1ae3d23eba506af913063" rid="357780:7906661" ref-type="bibr">9</xref> and Klotzl <italic id="emphasis-c86632eebebd976729c2c283f54557d2">et al</italic>., <xref id="xref-931f51b00d4a633b3517c19b7bdac5b3" rid="357780:7906659" ref-type="bibr">17</xref> demonstrated that the AR-positive is lower than in this study among RCC patients. Nine studies (1,447 cases) surveyed for AR expression in RCC patients in the meta-analysis and also expressed relationship tumor grades and pT stages with it. Zhu <italic id="emphasis-f265e06faed0770c804b43820d85a87e">et al.</italic>, <xref id="xref-fdc2f54bf7dadc4086637ce1b5f168b2" rid="357780:7906682" ref-type="bibr">11</xref> incontestable that AR expression rate was negatively accompanied pT stage and Fuhrman’s grade in RCC patients. Also, this relationship was indicated by Foersch<italic id="emphasis-3f0c5525a7d70ab58729ade5963b7779"> et al.</italic>, <xref id="xref-a2b1182e05f17353371dcdd02fa440e0" rid="357780:7906657" ref-type="bibr">14</xref> and Langner <italic id="emphasis-2a06c6dedd41f92163774ae96707e338">et al. <xref id="xref-0eb987e5d39d640ac461f4fa4d9d8ada" rid="357780:7906661" ref-type="bibr">9</xref>,</italic> Zhu <italic id="emphasis-320d09a550c8af8e19720c7feefc9734">et al</italic>., <xref id="xref-f880a4dfd53ff84770708cc7f002ed08" rid="357780:7906682" ref-type="bibr">11</xref> reported prevalence of AR had a negative relationship with pT stage and grade in RCC patients. Foersch <italic id="emphasis-919db4b20f799338f536d97f07e166b4">et al</italic>., <xref id="xref-f989017892778cb931594e8636573a9b" rid="357780:7906657" ref-type="bibr">14</xref> and Langner <italic id="emphasis-dc7900e7fc2ab2dd6bbe748f5ac09b00">et al</italic>.,<sup id="superscript-38"> </sup> <xref id="xref-0c23cd70415ac15da7da9b076b29497f" rid="357780:7906661" ref-type="bibr">9</xref> reported the results opposite of him. Noh <italic id="emphasis-12fce346a03cc6dfb30ca3ab05eb70ae">et al</italic>., <xref id="xref-bce0e7c5f8198fb15ebdae0e38af5454" rid="357780:7906668" ref-type="bibr">10</xref> said AR expression related with the histological nuclear grade (p&lt;0.027) and TNM stage (p&lt;0.002). The meta-analysis showed AR expression correlated with low grade and pT stage positively. </p>

      </title>
      <p id="paragraph-42">As our knowledge, this study is first meta-analysis to research the impact of AR expression on RCC patients. The current study showed the level of expression of AR and its relative with tumor grade and pT stage in RCC patients. Additional studies are needed to the role of AR expression in these patients.</p>
